Many thanks for taking the time to offer feedback on the GP-level data project.

The Department of Health and Social Care wants to give each GP access to their own (individual-level) data to help them to reflect and learn. By ‘individual level data’ we mean data mapped to the patients on your practice list registered in your name, via the usual GP field.

You have received a report of your individual data as the first phase of a project which intends to test the feasibility of extracting and disseminating individual level data back to GPs.

This survey asks you for your feedback, including how useful you found your individual-level report e.g. how useful are the selected metrics for your work, and how you have used them.

The results  of this survey will be used to inform the next steps for this project.  Please note that your feedback will be shared with NHS Digital, the Department of Health and Social Care and NHS England to help them evaluate the effectiveness of this work and to plan future work.
