The CSDS (Community Services Data Set) is a patient level, secondary uses data set which delivers robust, comprehensive, nationally consistent and comparable person centred information for people who are in contact with publicly funded community services.
Submitting to CSDS is a legally mandated requirement for all providers of publicly funded community services. Please see the bottom of this page for useful links and documents.
As part of our engagement with stakeholders, we have created this survey to gain knowledge and information with regard to Hospice providers.
YOUR views are important to us and will help us to provide the support that YOUR services require to be able to submit data to CSDS.
