Introduction

The purpose of this survey is to assess the benefits and/or disadvantages of the Additional Information to patients and carers. Please see the Overview section for a description of the Additional Information.

Please be reassured that the results will be used only for this purpose.

You are welcome to provide answers only to the questions of most interest or relevance to you, however, we are grateful for your feedback and would appreciate as much detail as you are able to give. We expect that the survey will take approximately 15 minutes to complete.

You are welcome to complete this survey on behalf of yourself or someone that you care for.

Please do not provide any personal information.

If you are happy for us to contact you about your responses, please provide your contact details at the end of the survey- we will only contact you about the survey.

 

Privacy information

Analysis will be undertaken by NHS Digital internal staff and results will be collated and anonymised before sharing.

By filling out this survey you agree that we will process your data in line with the purposes agreed above and NHS Digital's Privacy Notice.

We respect your trust and protect your privacy and therefore will never sell or share this data with third parties. Your data will only be used by NHS Digital for the purposes specified. We will only use your data for the purposes you have agreed. We will store any personal data for 6 months. After that point, all personal elements will be deleted.

You have the right to withdraw from the survey at any time. You also have the right to withdraw personal information once you have completed the survey. If you do change your mind about sharing your information with us, please contact bau.operations@nhs.net

Publishing responses (What Happens Next)

In line with the provisions of the government's Code of Practice on Consultations (available at   https://www.gov.uk/government/publications/consultation-principles-guidance), a summary report will be published within 12 weeks of the survey closing.

Publication may be on this website, elsewhere online and/or in printed form. A summary of responses will be published on the NHS Digital consultation website and this may include quotes of comments which however, will be non-attributable.

If appropriate, the summary will include a list of organisations that responded but not personal names, addresses or other contact details.

Note that unless we have asked for consent to contact you to discuss feedback, and you have provided that consent, we will not respond to comments on an individual basis.

Within 3 months of the survey closing we aim to publish a summary of responses recieved and provide a link to it from this page.
